Health Screening
INSTRUCTIONS FOR DOWNLOADING AND USING THE HEALTH SCREENING APP
- Go to the app store and look for “Activity Scheduler”
- See below – Icon has blue baseball cap with rST – may say “sports”
- Download the app and launch
- Look for state: New Jersey
- Type in Seton Hall Prep or look in alpha list
- Verify with a valid email
- Set up profile with name and “student” as member type
- Any issues with login verification email rod@shp.org
- Go to profile to enable the “remind me every day” notification
When using each day:
- Look for Daily Health Screening in the menu
- Default setting for “I am experiencing” statements is no
- If yes is submitted you will be pulled from school
- Enter your temperature
- Confirmation appears after “submit”
- YOU NEED TO SHOW CONFIRMATION SCREEN AT DOOR EACH DAY – this has your name, the date and a box stating “Cleared” THIS MUST BE PRESENTED AT THE DOOR EACH DAY
- You can access your confirmation screen by clicking on Profile in the top right and then selecting today’s date. This is what you will show at the door each day.
