[All athletes must file a doctor's physical form each year . To
avoid complications during the year, it is highly recommended that this physical occur as close to the start of school as possible. It is good for 365 days. Once it becomes 60 days old, the only form needed
is the parental medical update. [below]
Medical Update: for those Athletes whose annual medical
physicals already on file with the school are more than 60 days old when tryouts begin. Please return clipped together and all at the same time.
IMPORTANT NOTE: NO STUDENT MAY TRY OUT UNLESS THESE FORMS ARE ON FILE WITH THE TRAINER, INCLUDING ALL APPROPRIATE SIGNATURES.
